Procedure for referral

We accept direct referrals from parents although many are also made by G.P.s, and Consultants.

Once the referral has been made to the Practice:

Parents will be offered an appointment for initial assessment. A confidential questionnaire is sent to the parents plus one for the school (where this is relevant) to complete. We ask that this is returned in the SAE provided. This will enable the physiotherapist to have a clear understanding of the history and current concerns as well as removing the need to ask questions that could be awkward for the parents to answer in front of their child.

The assessment takes place in the most suitable place for the child and his/her parents/carers. This may be in the home, school or at the practice premises.
